How will my data be stored? In May 2018 the Data Protection Act was replaced by the General Data Protection Regulations (GDPR). The changes to the Data Protection Act are aimed at ensuring that your personal, confidential and sometimes sensitive data, is held privately and securely.

How long will you hold my information for? This depends on the organisation you are registered with. for example the NCH says “ You should keep written records stored securely for 8 years after the last interaction with an adult client and up to the age of 25 for a child under 16 when last seen or 26 years for a 17-18 year old.”

What if I would like my data to be destroyed before this date? Under GDPR rules, you are able to request the deletion of any of your records at any time.

Am I able to see or get a copy of the information held by you? In line with GDPR, yes, within 30 days.

What are your Reasons for collecting this information?  The reasons for collecting the information are to provide you with a detailed personal hypnotherapy experience. To comply with all the professional regulations of the NCH and ICO.

How will you store my information securely? Written information will be stored securely with a combination of paper and digital format.

Are our discussions within the hypnotherapy sessions confidential? Yes, unless I need support from my supervisor or I believe that you are about to harm yourself or another.

Will you discuss information about me with other health and social care professionals? Only with your written consent.

Who is the Data Controller and what is their registration number? I am the Data Controller and I can provide the ICO registration number if requested.

Terms & Conditions

By completing the Personal Details Form you hereby agree to all terms and conditions listed on my website and provided to you during our sessions.
Keep in Confidential - Our sessions are confidential unless I have concerns for your health and welfare or I have concerns regarding the health and welfare of another person. You agree to provide accurate GP details and update these when necessary should they change. You are giving your permission for me to contact your GP for any safeguarding concerns.

Supervisor - NCH guidelines require All Hypnotherapists meet with a supervisor on a regular basis. On occasions this will be to discuss direction of therapy for a client, however no personal names or addresses/ phone numbers will be disclosed.  
Payment - You agree to send payment via BASC a minimum of 24hrs before your agreed appointment date and time. For example if your appointment is at 12pm on Tuesday payment must be received by 12pm on Monday otherwise you give your permission for me to offer your appointment slot to someone else. Full Payment is required if the appointment is cancelled within the 24hrs prior.
